||Simple one Step resction procedure in circular constriction rings

Standard technique for the correction of constriction rings isan excisision of the ring ans a closure by multiple Z-plasties.
We now prefer to perform a complete resection of the ring combined with an extensive resection of the deep fibrotic fascias, a longitudinal fasciotomy, a mobilisation of the soft tissues and a circular primary closure.
Material: Between 2000 and 2003 we treated 86 patiens with a constriction band syndrome with 252 procedures in 126 operations. 16 extremities showed a complete constriction ring, located at the upper arm (1), forearm (4), middle hand (1), thigh (1) and lower leg (9).
In all cases we were able to perform a complete one step resection of the ring without any circulation problems. During operation we did not find any subcutaneos veins and nerves down to the level of the fibrotic deep banding. We did not have any wound healing or circulation problems.
With a follow up of more than five years we do not see any rebanding and no lymphedema, in all cases the circulation is good. So we continue to use this procedure as a standard technique for correction of constriction bands.
